Book an Appointment

Important New Patient Information:

​

  • Rates and information on insurance coverage is available here.

  • Schedule an Initial Assessment to get started. After you make your appointment, you will receive an email with a link to the patient portal to complete the intake process.

  • The following documentation must be provided at least 48 hours before your initial assessment:

    • All intake forms completed in the patient portal

    • Insurance referrals (if required)

    • Labs

    • Medical records

    • If not received within 48 hours, we will have to open the appointment time for other clients.

  • A credit card is required to hold your appointment.

  • At least 48 hours notice is required to cancel or reschedule an appointment. Your credit card will be charged $100 in case of late cancellation or no show (exceptions granted due to illness).

  • Pediatric Clients:  If making an appointment for your child, please schedule it in his/her name (not your own).​ We can only bill insurance if the child is in attendance.

  • Aetna clients:  Aetna requires that my services be "prescribed by a physician". Therefore, prior to your session, we must receive a referral from your physician that includes your ICD-10 diagnosis code(s). Sessions limited to 55 minutes unless you want to self-pay for the extra time. More coverage info here.
